Preoperative management of the patient with chronic kidney disease

Chronic kidney disease (CKD) is a pathology with an increasing prevalence worldwide. Its spectrum of manifestations is very wide and in advanced stages, it behaves like a multisystemic disease that associates high morbidity and mortality. The performance of surgical procedures in this type of patient carries a high risk of complications, in part due to a higher prevalence of cardiovascular diseases. Even in earlier stages of CKD, an increased risk of perioperative cardiac events and mortality has been observed. Therefore, in the preoperative assessment of kidney patients, the risks derived from the surgical procedure itself, as well as the specific risks of kidney disease, should be considered, in order to establish therapeutic strategies aimed at reducing their surgical risk. A bibliographic review of the scientific literature published to date is carried out, with the aim of understanding the pathophysiological phenomena that occur in this disease, and making the pertinent preoperative recommendations according to current evidence.
