We review the current recommendations for the management of the known difficult airway in lung resection surgery: we present a case with severe anatomical distortion of the airway that required pulmonary isolation.
In our daily healthcare practice, we are faced with the management of the expected difficult airway (VAD) that we solve with fibroscope-guided intubation of the awake patient (FBO), but its management in lung resection surgery has special relevance, since one-lung ventilation the required requirement leads us to know alternative techniques to the conventional double lumen tube, which due to its external diameter, rigidity and concavity, can be a challenge to place in cases of severe distortion of the airway such as the one we present.
According to current recommendations, we have devices such as the univent tube and bronchial blockers whose placement guided by FBO can help us manage one-lung ventilation in VAD.
