High spinal anaesthesia following failed epidural analgesia in urgent caesarean section. About a case report.

Abstract

We describe the case of a pregnant woman in whom spinal anaesthesia is performed for caesarean section due to inadequate labour analgesia.

 The patient had an epidural catheter for labor analgesia that was initially effective, requiring several boluses for pain control in the last stage. Despite not having administered a local anesthetic via epidural in the last 30 minutes prior to the performance of the caesarean section, having no adequate motor or sensory block and having reduced the usual intrathecal dose, there was a high spinal block with desaturation and the need for orotracheal intubation.

Given the complexity of managing this kind of situations and the importance of early diagnose and treatment of a high spinal block, the case report is described and the concomitant situations of this clinical scenario are analyzed.
