Intravenous fluid therapy evidence. An outstanding issue.

Intravenous fluid therapy is a key component in the protocols and clinical pathways in enhanced recovery after surgery protocols. This article reviews the new recommendations based on current evidence: from the limitation of preoperative fasting and hemodynamic monitoring of the systolic volume, to a restrictive but permissive balance in the intraoperative and rapid recovery of the oral route in the first hours of surgery. Perioperative patients with hypotension or evidence of inadequate tissue perfusion should first be fluid-optimized before implementation of vasopressor therapy.
