La rotura de músculo papilar secundaria a endocarditis infecciosa en una complicación rara, sobre todo cuando se presenta como la manifestación principal de una infección por un microorganismo agresivo como lo es el Staphylococcus Aureus que suele producir además destrucción de los velos y del aparato subvalvular.

Antero-lateral Papillary Muscle Rupture as presentation of Infective Endocarditis by Staphylococcus Aureus.
A 22-year-old man was admitted to our hospital for dyspnea and fever, requiring intubation and mechanical ventilation. Echocardiography showed severe mitral regurgitation with rupture of anterolateral papillary muscle. Mitral valve replacement was performed, intraoperative observation confirming the anterolateral papillary muscle rupture, with no evidence of inflammation in both leaflets or chordae tendineae. Histological and microbiological analysis demostrated methicillin-sensitive Staphylococcs aureus infection.
