For the understanding of the hemodynamically unstable patient and its treatment, a knowledge of the cardiovascular physiology and the available hemodynamic monitoring methods is required. In this review, the role of both venous and arterial blood circulation and cardiac output monitoring, especially by echocardiography, will be discussed. The review is structured in the following sections: venous, arterial and hemodynamic monitoring.
It is necessary to include already known concepts such as the average sistemic filling pressure (Psmf) and the elastance (E), both aortic and dynamic, in order to evaluate the response to fluid administration. In the near future the algorithms of the hemodynamic monitors will include these variables and many others that will provide us with a very useful tool for the management of the hemodynamically unstable patient.
