Abstract

-The DIEP flap consists of skin and subcutaneous tissue vascularized by a perforating artery that is anastomosed to the internal mammary or thoracic vessels.
-The anesthetic objectives are: normotension, euvolemia, normothermia, pain control, prevention of PONV and thromboembolic disease. In general, avoiding situations that produce vasoconstriction.
-The most frequently used anesthetic techniques are TIVA, OFA and balanced GA. Recently, regional anesthetic blocks have been used both as anesthetic maintenance and postoperative analgesia, the most relevant being TAP block.
-The most frequent postoperative complications are hematoma, partial and total flap loss and fat necrosis. Clinical observation, the use of the oximeter on the flap and Doppler can improve the flap survival rate.
