Fat overload syndrome after treatment of systemic poisoning by local anesthetics: A case report.

Abstract

The use of lipid emulsions as antidotes for local anesthetic systemic toxicity (LAST) has increased in recent decades. Fat overload syndrome (FOS) is one of its complications

We report a case of a pregnant woman who received a 2% lidocaine bolus for caesarean section through an epidural catheter. Due to the suspicion of LAST because of neurological symptoms, she received a load of Intralipid®. In postoperative period, she debuted with jaundice, hyperthermia, tachycardia, and lactic acidosis. Once other complications were ruled out, FOS was suspected due to the presence of milky plasma. Treatment with fluid therapy and antipyretics was decided.

Although FOS is a rare complication, it should be suspected in patients who have received high doses of lipid emulsions. There are no studies on this syndrome in the obstetric population and there are different opinions regarding its treatment, although the central idea is fluid therapy to reduce plasma viscosity
