Anesthetic challenges in a patient with gray platelet syndrome for cardiac surgery

Gray platelet síndrome (GPS) is an uncommon platelet fuction disorder (<1:1000000) characterized by macrothrombocitopenia and hemorrhages. There are not established guidelines regarding perioperative management in patients with GPS, so the management is based on a few cases reported. The recommendations published on literature to prevent perioperative bleeding in the described patient are the use of tranexamic acid, desmopressin and platelet blood transfusión before each potencial bleeding procedure. In these article we describe the management of a patient with GPS who need cardiac surgery to repair mitral valve prolapse.
