Preoperative fasting is defined as the period of time prior to surgery in which oral intake of solids and/or liquids is not allowed. It is essential in daily perioperative practice given the serious complications, even life-threatening, in which can result for non-compliance, as well as the consequent suspension of scheduled diagnostic-therapeutic procedures. We have reviewed the most recent updates of the international perioperative fasting guidelines with the aim of grouping and summarizing the latest recommendations. In these, the importance of avoiding excessively prolonged fasting times is emphasized due to their deleterious effects. Issues such as the ingestion of chewing gum or the distinction between carbohydrate-rich liquids, the administration of which is recommended given the associated benefits recently observed, are addressed. Finally, the persistent lack of consensus among societies on certain controversial sections, especially that relating to fasting in the pediatric population, is reflected.
