Reconstrucción mamaria con colgajo DIEP

-El colgajo DIEP consta de piel y tejido subcutáneo vascularizado por un arteria perforante que es anastomosada a los vasos mamarios internos o torácicos.
-Los objetivos anestésicos son: normotensión, euvolemia, normotermia , control del dolor, prevención de NVPO y de la enfermedad tromboembólica. En general, evitando las situaciones que produzcan vasoconstricción.
-Las técnicas anestésicas más frecuentemente utilizadas son la TIVA, la OFA y la AG balanceada. Recientemente se han usado bloqueos anestésicos regionales tanto como mantenimiento anestésico como analgesia postoperatoria, siendo el más relevante el bloqueo TAP.
-Las complicaciones postoperatorias más frecuentes son el hematoma, la pérdida parcial y total del colgajo y la necrosis grasa. La observación clínica, la utilización del oxímetro sobre el colgajo y el doppler, pueden mejorar la tasa de supervivencia del colgajo.
